2023 Important Changes to the Neighborhood House Annual Activation Process

In order to continue receiving Neighborhood House services, all clients must complete an Annual Activation process. This year, the process has changed to ensure we understand your current household needs and we have everything we need to maintain your active status.

Below is the new process for Annual Activation starting Feb. 1, 2023.

  • To maintain an active status with Neighborhood House, each household must complete the Annual Activation process during the month of the primary contact’s birthday. *If the Annual Activation is not completed by the end of that month, services will be suspended until the Annual Activation has been completed.

  • The Annual Activation process is now in person. You’ll meet with an intake specialist to review the activation materials and discuss current needs. Transportation service will be provided upon request. Please note: We’re no longer accepting documents through email. *Any exceptions must be approved, ahead of time, by the Director of Client Services. 

  • Walk-in activation hours will be held each week on Monday, Tuesday and Wednesday, between 10 a.m. and 2 p.m.

  • PLEASE DO NOT COME TO THE OFFICE UNTIL YOU HAVE ALL OF THE REQUIRED DOCUMENTS. The list of required documents and the Activation Form is below.

  • When you arrive:

    • Please sign in at the reception desk. 

    • Take a blank copy of the Activation Form. Complete this form while you’re waiting to meet with an intake specialist.

    • Reception will verify that all documents are available and the form is complete. Please note: if there are documents missing, we won’t be able to complete the activation. We’ll place your documents in an envelope, hand them back to you and ask that you return when all documents are available. 

    • Your documents will be copied and the original versions will be given back to you.

    • Reception will notify you when the intake specialist is ready to meet.

    • The intake specialist will discuss your Activation Form, required documents and the current needs of your household. During this discussion, they’ll update your folder and your records in our Client Service Tracking System.

    • At the end of the discussion, if everything is complete, your account will be active within 2 business days and will remain active until your next birthday month.
